• Reslease for Administration of Medication at School
• Locker Agreement - signed by Student & Parent/Guardian
• Internet License - signed by Studnet and Parent/Guardian
• Athletic Physicals/ Permission slips
• Proof of residency if you have moved
• Completed Life-Touch Picture Form
• Volunteer Informaiton Sheet

Students May Pay for the Following :

• Money in Lunch Account
• Fall Sport Fee ($25.00)
